Community Engagement Navigator

Position Overview:
The Community Engagement Navigator plays a key role in helping clients explore and engage with their communities in ways that are safe, fulfilling, and aligned with their individual needs and interests. This role supports vulnerable populations—including seniors, individuals with disabilities, and those managing chronic conditions or mental health challenges—by promoting social connection and access to community-based resources and activities.
Key Responsibilities:
Build trusting relationships with clients while honoring their preferences, values, and goals.
Act as a liaison between clients and community services to increase their comfort, independence, and participation in community life.
Provide one-on-one support to clients by:
Researching and visiting community programs and resources
Assisting with appointment scheduling and transportation coordination
Accompanying clients to events or appointments as needed
Encouraging engagement in activities such as day programs, libraries, community centers, religious services, or local events
Maintain clear documentation and communicate client concerns or changes to the office team promptly.
Collaborate with internal staff and external service providers to ensure coordinated, respectful care.
Qualifications:
Demonstrated understanding of patient-centered care
Experience working with vulnerable and culturally diverse populations
Familiarity with local community resources and willingness to research new ones
Strong communication and interpersonal skills
Able to set and maintain professional boundaries
Critical thinking and problem-solving skills
Ability to work independently and as part of a team
Working knowledge of Microsoft Office and comfort with technology
Valid driver's license and access to an insured, reliable vehicle
Commitment to maintaining confidentiality and following HIPAA regulations
Preferred Education & Experience:
Minimum 1 year of experience in healthcare, mental health, social work, or community services
Degree in social work, psychology, sociology, public health, or a related field preferred but not required
