Location: Greater Boston area
Schedule: Flexible (some evening/weekend work may be required)
Compensation: Based on experience

💡 What’s this role all about?

Are you passionate about helping people transition into independent community living? Do you thrive on connecting individuals with real-world resources, day programs, and social activities that match their personal interests and goals?

As a Community Engagement Navigator (CEN), you’ll be the friendly face and guiding hand for individuals exploring life beyond institutional care. You’ll help participants in the MFP (Money Follows the Person) Demonstration Project experience firsthand what living in the community can look like—on their terms, and in alignment with their goals.

This isn’t just a job. It’s a chance to help people reimagine what’s possible in their lives.


🧭 What You’ll Be Doing:

  • Empower participants to explore community living and make informed choices.

  • Build trust by learning about their histories, interests, and goals.

  • Identify & explore local resources like day programs, religious centers, grocery stores, and community groups.

  • Accompany participants on visits to community locations to ease transitions.

  • Collaborate closely with the interdisciplinary care team to support person-centered planning.

  • Promote social connection by matching people with opportunities that bring them joy.

  • Document visits and progress in a timely and accurate manner.

  • Act as a bridge between clients and local resources—because sometimes, it takes a Navigator to discover a new path.


🎯 You’d Be a Great Fit If You:

  • Have experience supporting clients with social and community engagement.

  • Hold a BA in Human Services or a related field (preferred, not required).

  • Are creative, compassionate, and a strong communicator.

  • Know your way around your community and have ideas for engagement.

  • Are tech-savvy and comfortable with documentation and communication tools.

  • Can work flexible hours and navigate around the area (preferably with a car).

  • Believe in dignity, choice, and self-determination for seniors and people with disabilities.


🔧 Bonus Points If You:

  • Have experience in home care or human services.

  • Can think on your feet and bring a little humor or heart to your work.

  • Are comfortable lifting up to 75 lbs and providing occasional physical support.

  • Know how to find the hidden gems in the community—be it a quiet walking trail, local café, or social group.


🚗 Transportation Note:

This position involves local travel, including home visits and community excursions. You must have a reliable, insured vehicle ready for daily use.
